Start days ahead, not on the morning itself. Leave a carrier open in the living room with a blanket and the occasional treat inside, so it stops being the object that only appears before the vet. A harness goes on for two minutes at a time indoors, with food, and comes off before the cat objects rather than after.
A carrier belongs on the floor behind a front seat, or on a seat with the belt run through its straps — never loose, never on a lap. A cat that gets out mid-journey is a danger to itself and to the driver. Draping a light cloth over it helps: most cats settle faster when they cannot see the road moving.
